I'm going into Rads and would really like to avoid the hassle of juggling advanced positions and prelim positions if possible. Is there any major disadvantage to applying to only categorical programs, assuming I like all the programs I apply to? Has anyone else heard of anyone doing this? Thanks!

I have heard of my peers doing this although they claim to have exceptional Step scores (and presumably an above average transcript) with good research and EC stuff. I do not think these people I know about will have an issue, but they also do not have any geographical ties and are applying everywhere.

I am not entirely sure what the ratio of C to A programs are for radiology, but that would be the first statistic to look into for all the programs you are considering. If the programs you apply to have more A than C positions, then you would be at a disadvantage. This would give you a base statistic at least without taking your application into account.

That then bring up how competitive your application is.

On the other hand, though finding a PGY-1 year is more work, time, and money, applying for C and A positions of the same program will not cost you any extra work except clicking another button.

It all comes down to what you are willing to risk and how confident you are with your application.

This might not be super helpful lol, but at least I do know some people who have successfully done this. (of course n=2 so take it as you will.)
