Do all American PGY-1 and PGY-2 programs go through the match (NRMP or SFMS)? If so, is there an accreditation body that requires this?

No. Some small programs may decide not to use the match, and I am not sure of where you would find a list of such programs, but would guess its an extremely small number. Programs are not required to participate in the match but must offer the majority of their positions through the process if they do so.
