I think you are implying that disadvantaged applicants are expected to later give back to their community by being more likely to practice in low-income areas, etc similar to the URM reality. In a few URM threads that has been refuted. Those raised in economic adversity are not more likely to practice in underserved areas in the same way that URM physicians are more likely to serve underserved communities. It is one of the reasons that adcoms and schools do not just consider SES in applicants.
Hopefully someone will find a study to discount it. On a state-by-state level, however, it is possible that rural programs enroll a higher number of disadvantaged students that later go back to their community.
