T20s and primary care focus

I'm aware that all medical schools match students into primary care residencies, but would an applicant sharing that they're strictly interested in primary care make top schools look past them (since these schools tend to produce specialists)?

I'm interested in internal medicine because I want to work with older adults, but I also want to go to a T20 because the PI I've done geriatrics research with is at one of these, and I want to continue working with them.

It depends on the school.
WashU St. Louis matched zero graduates into FM residencies last year because they do not encourage it.
If IM is primary care enough for you, they did match a lot into IM.

The T20s want to train the next generation of academic medicine's leadership and there are roles in academic medicine for primary care providers who are into teaching/research/clinical care/advocacy. General internal medicine and general pediatrics are considered primary care as well family medicine. Some geriatrics is primary care and some is a consult service model.

I would not expect an adcom to dismiss your application if you specify that you are interested in geriatrics, and, in particular, in research as well as patient care.
