Are fellowships part of Return of Service?

So lets say I do 5 years of Gen Surg...then 2 year fellowship of Surgical Oncology or Transplant Surgery...

Do I have to do 5 or 7 years of Return of Service?

and can I do the RoS as a Transplant Surgeon (or whatever my fellowship is in), or does it have to be in General Surgery?

It depends what your contract says.

If you sign a ROS for general surgery, it will 1) stipulate a date by which you must beginning returning service (usually within 6 months of your residency completion date) and it will 2) stipulate the field in which you serve e.g. general surgery.

If you really want to do a transplant surgery fellowship, make sure that you negotiate that into your ROS contract e.g. my commencement date is 7.5 years from the beginning of my general surgery residency, and I can return service as a transplant surgeon. Whether you have to return service for 5 or 7 years depends on how the contract is written.

If you don't get it in writing, the health region is not required to alter the contract to accommodate you (and likely won't). Remember - it's a contract. If you can't live with it, don't sign it!

The fellowship will not be part of the ROS i.e. You'll only need to serve 5 yrs. That said, to postpone the ROS you'll have to contact the PPO and have it negotiated into your contract. NB- As far as I know, this only works in Ontario- definitely not Nova Scotia or Sask.

I'm not sure when and where you signed your ROS; and I think we should mention that the contracts are unique to each province.

The contract in Ontario last year didn't really have any option to negotiate. The contract wasn't even provided to us until late May long after the match. It was a standard form contract. The current contract doesn't use the language 'Residency' it just says 'training period'. It also doesn't mention practice specialty or define what an under-serviced area is.

The bottom line is that very few specialist have finished residency with an ROS so far. They didn't exist in Ontario at all before 2006 or 7 as far as I know. I'm anticipating it will be a big mess once there are substantial numbers of specialists with ROS contracts the MOHLTC will have to place. There were huge changes to the agreement when the first large cohort of FM docs with ROS contracts finished two years ago.

Sorry for the slight hijack...I know the ROS agreements are province-specifc, but does anyone know generally if military service can count or be worked into the ROS? They are a 'under-served community' in many ways...

Just curious. Are there any provinces which do not have a ROS clause? 😛
