Are letters from every PI that you've ever worked with required for applying to Md/PhD programs?
What is you've worked with four different ones since high school (some more meaningful than others)? Do u ask each of them to submit to all your schools when you apply?

If you've worked in 2 or less labs I'd say yes, 3 might be ok, but after 4 I feel like it's overkill and they get the point..

I thought that was a firm requirement that they wanted to see letters from every single person you've worked with.... lol is that not the case?

You want backup letters, strong letters, and reference letters. Use your PI's to your advantage.
And... Nope, you don't have to turn-in letters from everyone you've worked with.😉

So on your app, even if you list the list of labs you've worked with, its not necessary to include letters from all. (and that is OK with med school adcoms?)

If you've worked with anyone for a significant period of time, you are expected to have a letter from that person.

Schools won't care if you don't submit LORs from summer research experiences, but anything that lasted one academic year or longer generally warrants a recommendation. For most people, this will amount to 1-3 research letters.

Sure it is.
However, you always want to bring big guns and a great deal of ammo, right? For example: 2 years ago I applied for certain position and I turned-in a total of 32 letters (12 were the minimum). 🙂

It isn't necessary, and most schools won't ask for letters from pre-college research PIs (I did some middle school/high school, too). Make sure you have them from researchers you've worked for recently, though--summer internships, academic research positions, or independent study/thesis work.

Some schools, such as UPenn, will specifically ask for a LOR from every research advisor you list. If you don't have one, you have to state why, so it's safer to just get letters from each PI you work with. You can choose which letters go to which schools via AMCAS.

Don't do this for medical school. No one wants to read 10 LORs for a single applicant.
