I hate to disappoint you but your mom clinical volunteering does not show any competency in service orientation. It does a great job showing your mastery of engineering or science principles instead. But with no or few service orientation activities showing you can be comfortable in a humble way to alleviate distress, your application can be screened out at many medical schools.
I don't know if engineering- focused programs like Texas A&M EnMed or Carle will overlook this, but in a pool where most others have something, it will be a challenge to stand out here.
PS, FWIW, you have to report those IAs... I think most will review it and not consider it as something to worry about, but you have to disclose what you should. Make sure the conduct offices help you craft this disclosure statement.
