In the "Disciplinary" section there's a question: "Have you ever had disciplinary action taken against you by any professional society or professional association?" I was on academic probation and then academically withdrawn from my freshman year college. Would that count as a professional society/association?

I don't think so. I think that it is more like an organization like ADA, AGD, etc. Those are professional organizations.

No. This section is talking about disciplinary action taken for misconduct. If you were put on academic probation for grades and then withdrawn that doesn't apply here. I believe what they are asking is whether you were kicked out a professional society or professional association. Examples of this would be a lawyer that was disbarred by his or her state's bar association, a teacher that was dismissed from a union related to school concerns, or a journalist that was dropped by a press club or association due to questionable ethics in reporting. If there were other reasons other than grades that resulted in being placed on academic probation (ie run in with the campus police) there are places on the primary application through AADSAS that talk about that.

OK, well it sounds like you're in good shape then. Good luck with ASDOH. Kirkville isn't a bad little town. I actually wouldn't mind going to school there at all.
