asdoh asks for the address of 2 recommenders. for those that sent a committee letter, did you write the committee's address or the individual professor/dentist's address? thanks.

i put the professor's name and address down that was head of the commitee... the professor that signed my committee letter. a dentist letter was included in my committee letter and asdoh told me to put n/a for the 2nd recommendation because they would see it in my committee letter.

asdoh told me to put down the names of the two letters that I thought were the most important. I put a professor and the dentist I got a letter from. They told me to do this regardless of my Committee letter.
