Hey, I just got my supplemental from ASDOH. On one of the sections it says to list 3 evaluations. One from science teacher, another from a dentist, and a third from "supervisor, manager or director of a volunteer broad-based community service organization where you have significant participation".


Do I have to submit the third one because I don't have a letter from any of those?

Yes -- and it can be uploaded through Interfolio or mailed.

For the science professor and the dentist, you just need to provide the name of the individuals so they can look at these letters on your AADSAS.

Can I mail the letter myself or does it have to be mailed through the office?

Also do you know of any other schools that need similar LoR from volunteer programs?

I would provide whoever is doing the letter a pre-addressed envelope and have them mail it. I would make sure that they sign the back of the envelope where it gets sealed just in case. You don't want to be in a position where they think you read the letter or anything, since you should be waiving your right to review the letters.

MWU-AZ has a third letter that they say should be from someone "who can testify to the integrity and ethical standards of the applicant" -- not exactly the same as the community service one but still similar. That's the only one that I know for sure. I am sure there are others, but I am only familiar with the ones I applied to myself.

Yeah I just put all of my volunteer work -- including the ones I had on my AADSAS
 
Sorry, don't mean to hijack the thread but didn't want to make another...
For the Clinical Experience part where we list the actual clinical stuff, it says to list paid experiences. I don't have any and I can't submit without something there.. Do I put shadowing there or something then?? Cuz you can't put shadowing in the other two sections.
 
Same issue-- what I did was put "None" for all of the text options, the start and end date as today's date, and then 0 for hours. Not sure if that's the way to do it officially but it let me save it without giving me a hard time lol

I looked through some old threads and people put the same thing, just N/A instead of None.

Hi does the secondary ask anything about post bacc programs or grade updates? Bc I just got accepted into a post bacc program but I didnt have it on my application. What should I do?

It doesn't ask for that information. I would probably call the admission offices of the schools you applied to and notify them of you enrolling in a post-bacc and will be sending grades from that institution as part of your academic update.
 
I don't really understand what they mean by this question.

"Explain any prolonged periods of non-academic activity during or after full-time enrollment in formal coursework".

Do they want me to explain what I did after I graduated?

Yeah -- they just want to know what you did after you graduated if you aren't trying to matriculate right from undergrad.
