So under the "CLINICAL VOLUNTEER AND COMM SERVICE SECTION " you would assume that you should put all your community service and volunteer experince. BUT below it says list the ones that you were financially compensated for? How do get compensated for volunteer and comm service?!?

I just looked at it. Isn't there a section right below that for noncompensated volunteer work? That's what I saw atleast.

Also, is there pretty much no place to list observation/shadowing hours? It seems every section says to not include it. What do you guys think?

i just listed 3 volunteer experiences at the bottom. it says to not list shadowing, so i just put N/A in all the other spots.

Also, under the Statement of disciplinary actions part on the ASDOH supplemental...the question says..."Is there any information that is relevant to your ability to complete the A.T. Still University - Arizona School of Dentistry & Oral Health program and be eligible for licensure or employment that the University should consider?"

What does this mean? I don't know if i should just put NO and move on, or what. any feedback?

They want to know if you can complete the training and then get licensure....

If you have to serve jail time in the coming years...you won't finish. If you have a felony, you may not get licensure. Those types of things. You better be honest though...that is legally binding. They can sue you for unpaid tuition dollars if you leave the program mid way due to a previous sentence, etc...

For most people, you just gotta write no!

also...for the essay questions, is anyone's near the 500 word limit? both of my responses were more like decent-sized paragraphs. 500 words is almost a page. my answer to the community service question was probably around 200-250, and the ASDOH one was around 150-200. i couldn't really stretch them any longer, i wrote what i felt was important.

They told to provide informations for Evaluators.. one from dentist and another from the science teacher. What should I write for "degree" "Department" "college" and "title" for the dentist?
Do I just write the name of the clinic under department?
I am confused...
Also, what do I write if I don't know the "Degree" forthe science professor?

for degree...i put that my dentist was a d.d.s. i said N/A for department. I said the university he graduated from. title, i just put dentist. also, with the science professor, try to find their website online through their university. my professor had a website, and he was a PhD. hope that helps

maybe one of you can help with this, since we already sent our letters to AADSAS (committee and dentist) do i just use those same two letters and fill out the info on the secondary? or can i send in two different letters and then they have four letters? or are they just asking "which two letters do you want us to have before we consider your app complete"?

i talked to the ASDOH admissions office...the guy said for that section, you write down the 2 letters you want the committee to focus on. They will look at any other letters you send it, but they will focus the most on the two you write down in the supplemental application.

Oh so we just put down the information of any two LOR writers? or does ASDOH has its own evalutation form/letter? I havnt gotten LOR from my major advisor but can i put down her name under evaluation on ASDOH supplemental & get the evalutation form from her? I am so confused....I think ASDOH has like evaluation form or something that they want to fill out....which is something different than LOR. Help!!
