Asking for Letters of Rec

Hello,
I graduated in May but I am taking a 2-3 years gap years. I wanted to get my letters of recommendation now but I don't know what to include in my requests. I'm requesting letters from:
-My professor who was my adviser and taught 3 of my classes (all bio classes)
-A professor that I TAed for (a neuropsychology course)
-My ethics professor
-My adviser for a group that I was the president for and who helped me create several mental health initiatives for the campus

My problem is that I'm not sure how to request these via email. I'm on the other side of the country so I can't meet with them face to face. In addition, people tell me to include a copy of a personal statement but I'm not applying for a while so I do not have one yet.
So my question is, what should I say in the email and what do I include?

Things I included when asking for LOR:
-a draft of my personal statement. It's nowhere near what my actual PS was, but it gave them a general idea of who I am on a personal level. Definitely include this, doesn't have to be the same as what you're sending to AMCAS (but shouldn't have contradictory information that would make your letters seem weird)
-my resume
-a heartfelt email about what I'm applying for, why I want them to write me a letter, etc
-make sure to mention that it is a large favor you're asking and you completely understand if they are unable to. Don't assume they owe you one

That's all. I wouldn't include anything about deadlines/interfolio/etc until they say yes

I would include;
My transcript (current)
My resume
A DEADLINE. This one is most important. Tell them you need it by a specific date or it will be stuck in their inbox forever.
