Assigning different letters to medical schools

If I was designating letter 1 to school X and letter 2 to school Y. Would school Y be waiting/know about letter 1?

Essentially can you send different schools different letters without them delaying your application waiting for a letter that's on your AMCAS application, but not designated to them?
