Does anyone know of any good rating scales or patient questionairres to assist with diagnosing bipolar disorder and ADD in adults, in an outpatient setting?

I bet you could make a questionnaire based upon the symptoms.

Though the questions would be so transparent that one may as well ask a sophisticated client 'what disorder do you think you have?'

Wouldn't a psychiatry text book provide text book questions to ask on making differential diagnoses? Or the little blurb at the start of the bi-polar section would provide information about differentials.

You might have more luck with asking on the clinical psychology forum (psychologists being into psychometrics and testing including diagnostic tests such as the MMPI etc)
