A 65 yrs old man has nonhealing foot ulcer, impotence, and symmetrically decreased
sensation in the distal regions in a glove and stocking pattern. Which of the following pathologic findings is
most likely to be present in the peripheral nerves?
A) Acute inflammation

D) Axonal degeneration

I think E; that's the "dying back" phenomenon of symmetrical axonal loss from distal to proximal sparing the motor nerves seen in neuropathies 2/2 metabolic causes.
As for the exact difference between D and E, I don't know. What's your source for these?

for most of my questions the Source is Miracoli's world, not U world 🙂
 
The symptoms are characteristic with diabetic neuropathy especially the phrase "glove and stocking" because that is the tell tale sign of diabetic induced neuropathy. The foot ulcer is just icing.

If it were axonal degeneration due to an infection, lets say from the foot ulcer, then it would be sided but this is symmetric.
