Hi,
I was wondering if those schools simply require letter of recommendations where the professors write a "letter". Can you please explain how that works.
OR
they require the professor to fill out 'evaluation forms' meaning they rate the applicant and answer specific questions.

Which ones do these schools require?

AZCOM requests that actual letters by professors and a physician be submitted... no evaluations or ratings involved. I cannot comment on the other schools.

AZCOM is the same as CCOM, a letter from Science faculty and a DO/MD letter.

I think PCOM requested a pre-health committee letter which I guess sums up your science faculty letters and a physician LOR. If you do not have a committee letter, you need to acquire a physician letter and a letter from your Dean of the college you are attending or from which you graduated.

I'm not quite sure about NYCOM...My primary was there and some people told me to have letters sent: 2 science LORs, 1 non-science LOR, and a physician LOR. I think a pre-health committe would send all of them for you including their cover letter.

You should check the AACOM website about the schools requirements and the school websites themselves to make sure.
 
NYCOM and NJ ask for individual LORs not fil out forms. Regards
 
Another AZCOM note:

I remember a note on the application that they'd only accept the first two letters, and discard all the ones after that. I don't know if that actually happens, but since it requires fewer letters than most schools and the same package of letters is often sent to all schools, make sure you only send the two you want read!
 
when do they request the recs? do you send them in with the secondaries?
 
I had my recommenders send the LORs directly to the schools while I was completing my secondaries. You do NOT send them yourself. Either your pre-health committee office or the recommenders will send the LORs themselves.
 
Nycom only requires a letter (no form) from your undergrad school premed committee (or advisor). If that's not possible 3 letters from instructors (2 science, 1 other) will do. They strongly recommend a letter from a DO but it is not required. I would send them only after they have invited you for an interview because that's when the secondary is done.
