Bad undergrad GPA?

I am a first-semester student at Rutgers as a junior. I did my first two years of undergrad at Seton Hall and was really unhappy and had a few personal incidents come up, and ultimately, I did very poorly and by the end of my second year I had a 2.79 GPA. I am now at Rutgers and am studying harder. Please, what steps can I take towards bettering my chances at getting into medical school?
 
Get better grades so you can increase your GPA to over 3.0 . The higher the better. You also need 50 hours of physician shadowing, including primary care. Also accumulate 200 hours of clinical volunteering with patient contact.
