Has anyone ever gotten into every school that he or she applied to? Certainly not me, that's for sure...
Early Decision Program. You get to apply to only one school, and you hear from them before October 1st. If you get in, you are obligated to attend there. If you don't, then you can apply to other schools.
I was just wondering, because it seems like even the top students that are in the top 1% of everything get rejected from a few schools.
I know a guy who applied to 10 MD/PhD programs (9 were in the USNews top 10), his only rejection was from UCSF which accepted him for MD-only.
EDIT: I should also add that he accomplished this while wearing a sport coat and khakis to his interviews instead of a suit, and that he had never done a minute of volunteer work in his life.

And if the streams ever crossed, well... god help us all.MD/PhD is a whole different animal. We don't even let the applicants mingle together in the same room. The review process is different, the interview teams are different and the grounds for admission decisions is different.
