I interviewed at a program where they stated that one of their positions was funded by industry and thus your name may be added to the sunshine act database.

Is there any downside to this I should be aware of? I’m just not sure of the implications of this.

Yeah, mine says I got $50,000 from a device company, Medtronic I think. No one has ever brought it up to me though and I don’t think patients in my area are checking that.
 
Most places get money from devices companies to supplement their fellowships, with one fellow being assigned to that. It's generally not a big deal or taxed. In the future that may change, but I'm not sure. It's primarily meant to expose conflicts of interest but there's a fear that it may lead to regulators targeting it as income. In this case, the funds don't go directly to you, so it would be under the guise of research/grants, and should have zero impact on your future.


The other caveat here is that you can't be reported if you don't have a NPI AND a full medical license, at least until you finish when you get your full license.

If you're on there for a few hundred bucks from going to a dinner or two I doubt anyone would care.

There are people on there with hundreds of thousands of dollars. That will raise some eyebrows.

1) It is public record
2) Some institutions may query your data and could have employment implications
3) Patients may query and come to inaccurate conclusions
4) Attorneys may query and paint an unfavorable picture of you
5) Quite often the posted data is erroneous - I check every year and 3 of 7 years it has been wrong

Main advice, just make sure you check during the review period and dispute any inaccuracies

Plus, why would they make one person take the fall for that?

Let’s say a program has six fellows all getting paid 100k, and 4 staff each getting paid 500k. So that’s 2.6 million to run the program. If they then say we need this 100k from industry to fund your position, they should divide that burden between 10 people so each get a sunshine act reporting of 10k each which is not much and wouldn’t raise any eyebrows. But if they make you take all the weight, you will get reported 100k which I think may raise flags.

Different places do this differently. Some will split it up across the fellows. Others will assign it to one specific fellow. It's sometimes administratively easier to put one person as the industry grant fellow, etc, but you're correct that ideally it would be spread across the fellowship cohort.
