Would't you taper it to lorazepam or anything with a lower potency than that (diazepam), since the dosages for it are higher than for xanax, so you can manipulate the doses more easily?

Does anyone have a good reference for tapering someone from Xanax to clonazepam?

There's a insurance company that is no longer paying for xanax, so we are going to have to switch them to something it will pay for (clonazepam). This will affect multiple people so unfortunately no case will be the same. I didn't know if anyone has had experience with this. I will need to get the clonazepam close to steady state before xanax can be completely DC'ed or that patients will have withdrawals.
That's outrageous that they'd make you go through these hoops. The stuff is dirt cheap anyway, why bother cutting their formulary just to cause problems. Is clonazepam the only benzo they're covering? Clonazepam and alprazolam are basically equal in strength. Depending on the dosage you can switch the patient over in one or two steps. If the dose is larger than 2mg I would switch my patient in at least two stages every few days.
Duration of action is much longer with clonazepam so dosing can be done once or twice a day.
