Best = most surgical volume
Worst = least surgical volume; most clinic scut work
One thing that you can't "fake" in real practice is your surgical skills. And it is best to hone your skills (as much as possible) during residency and fellowship when the stakes to your reputation aren't as high. When you interview for fellowship, if surgical numbers aren't explicitly given to you, then I would be wary of that program. Do your due diligence and find out the current and past fellows' surgical numbers. Most of the better programs will tell you outright what their surgical #'s are because they are proud of them. Conversely, the weaker programs will try to hide their numbers or come up with excuses why surgical volume "doesn't really matter."
