Any thoughts, with the rising problem in health care in the US these days, like decreasing salary, nurses acting as doctors which leads to decreasing quality of health care, any countries where it would be better to practice in?

The general premises of your post may be skewed. It is a arbitrary statement that care decreases as nursing increases. As such it is a completely personal decision as to what countries are the best to practice in. Financially, American doctors still do considerably better than their counterparts in most developed nations.
