You aren't going to like hearing this: the best subspecialty is the one in which you display the most passion and interest. Don't pick based on perceived lifestyle, finances, or prestige. You'll only end up finding disappointment.
there are only a few subspecialties within derm. general dermatologists can do general, cosmetics, and phototherapy... so if you want to do those 3 things plus mohs you should do a mohs fellowship.

It would be possible to do all of those if one did a procedural fellowship.
The only question would be phototherapy. It depends how in-depth you mean. If you're just talking something like nbUVB then sure. If you start talking PUVA, UVAs, PDT, etc, then one may want more training somehow (not sure how) or just really pay attention to it during residency training (assuming the residency program has phototherapy).
The main subspecialties are Dermpath, Peds, Mohs/Procedural. There are others, but you have to search them out more intently, and they will not be ACGME.
Harvard's Wellman Center for Phototherapy has a fellowship
If we keep this up we will eventually need a fellowship on punch biopsies.
