Best IV medication book?

What is the best IV medication reference that has dosing, flow rates, IV push calculations, which meds go with NaCl/dextrose, etc? I just read something about the high rate of IV medication errors in the pharmacy and I'm scared to death because I will be training as a pharmacist in the inpatient setting soon! I don't think I got enough IV calculations and IV medication education during school. :scared:
 
Trissels book is good for IV compatibility. Not sure if it has calculations. I guess any pharm calc book would be good for calculations. I used Zatz book when I was studying for boards.
 
Most hospitals have "sets" they use to order meds so that you dont have to create the wheel.....

Trissels is good....

for math, you just have to understand how to do them....I learned some in calculations class and have used my pure math knowledge to get the rest.

LexiComp and package inserts can tell you things too.
