Would a Bier Block provide adequate anesthesia for hand/finger/wrist fracture repair? I know Bier Blocks are only good for surgical procedures less than 1 hour but for the sake of argument let's assume our world-renown hand surgeon can finish a fracture repair within the that time.

Bier blocks for fractures of the lower end of the upper extremity are technically difficult: You need to start an IV very close to the fracture and you need to wrap the fracture with esmarch bandage before injecting ( very painful).
So, Why not do a real regional anesthetic?
