I got stuck going through threads on the internet about Forrest tennant and came across this unbelievable claim. Thinks overdose deaths are being mislabeled, and chronic opioid users are having adrenal crisis or some ridiculous medical explanation for sudden cardiac death …. I have no words.

he needs to make more and more outrageous claims to justify his ridiculous position.

similar to certain politicians these claims are best left ignored, and certainly not amplified

- chronic pain does not cause sympathetic overload.

- his hypoglycemia claim is just uninformed. it is actually a complex interaction.

of note:

This review shows that the vast majority of studies report that opioid stimulation increases blood glucose levels. In addition, plasma levels of the endogenous opioid β-endorphin rise in response to low blood glucose. In contrast, in hyperglycaemic baseline conditions such as in patients with type 2 diabetes mellitus (T2DM), opioid stimulation lowers blood glucose levels. Furthermore, obesity itself alters sensitivity to opioids, changes opioid receptor expression and increases plasma β-endorphin levels.

- finally, opioids cause adrenal suppression.

Don’t let evidence get in the way of a good story.

Last month the local police dept. called my office requesting me to sign a 'death certificate' coz I was the prescriber of a patient's tramadol and gabapentin found at his home

He’s on staff with that “paper” as well.

These people really aren’t making the arguement they think they are. We are talking about prescribing high dose opioids such that when a patient is in fear of loosing the medications they commit suicide. Not the argument for opioids these people think.

they call themselves "pain management specialists" - what is your definition? because they can designate themselves as such, as pain societies have not restricted use of this term to those who are only fellowship trained.

or they may have taken alternative boards or "grandfathered" themselves into that title.

when you look at the CMS data - he prescribed 3168 opioid prescriptions, and 68% of his prescriptions to medicare were opioids.

id also like to mention, but it will get ignored by the lay public, that he has stellar online reviews of 5 * on google and 4.4* on Vitals.com

I'd call him an Internist who specializes in writing high dose opioids and controlled substances.

Obviously titles, degrees, and level of training don't count for much anymore in "medicine", but you asked for my definition:

A "Pain Management Specialist" is one who has graduated from an accredited residency and an accredited Pain Management fellowship.
