"Biggest Challenge" Choices- Appropriate?

Sounds like the first was a much bigger challenge to me. Which do you think was a bigger challenge? I don't think it matters if your biggest challenge was in high school; it's not as if you have any control over when your biggest challenge was. My advice: be honest. But think hard about what made it challenging and what you learned from the experience.

yeah... i don't think the one with your drunk friend would be appropriate...

It seems like with the first option you could really elaborate a whole lot more than you could with your second option. Tearing your ACL and doing rehab/training for 6-7 months sounds like it impacted your life more (in regards to time and the effects it had on your mind/body, etc.) than one time you called for medical assistance for a drunk friend. And though you said you weren't drinking at the time, I would still question why you were underage and with someone who was also underage and so intoxicated. Ya know? I think the second one would just open up too many questions.

With the first option you can really highlight how you overcome obstacles when something bad directly affects you.

I wouldn't worry about the stories being from high school. That's fine

I would not use the drinking story, if for no other reason than it doesn't really seem like a challenge. Maybe a fleeting ethical dilemma or something like that. I don't think that something that was resolved within a few hours one night is really a challenge
