Would it be acceptable for a licensed psychologist to provide CBT services for insomnia under the supervision of a physician, as part of the patient's sleep evaluation, and the visit be billed under the physician as a clinic visit? The visit would be specifically for the CBT.

No. This has nothing to do with physician supervision. You need to get your own contract. I know. Insurance companies suck.
