I was looking at the fee schedule on pain rounds site and am still little confused about billing. Could someone explain to me step by step how billing works. Maybe use a sample procedure something that is common in practice, for example like medial branch block at L3-L4. Sorry if this has already been answered.

Also what is the average hours, average salary, and malpractice cost

you might want to ask amy mowles from www.mowles.com, since she was the one who put this table together. I had the same confusion too but she explained the table to me at a conference. Sorry, I am not sure I can reproduce her explanations.
