The Phosphorylation of ATP from ADP and inorganic phosphate produces 2 ATP molecules for every 4 phosphate molecule.

Is this statement true?

If not, how many phosphate molecules are needed for 1 ATP from ADP? Just 1?

atp = adenosine triphosphate
adp = adenosine diphosphate
amp = adenosine monophosphate.

4 phosphates = 4 atp
