For my first two I picked my research (nutrition based) and volunteer tutoring with low SES students, both of which I'm passionate about. I'm having a hard time picking between these last two. I'd say I'm more passionate about the internship, but I'm afraid of not having anything explicitly medical chosen, so I'm wondering if it would be safer to pick hospital volunteering.

Do the BME. A whole lot of hospital volunteering gigs are pretty meaningless and not especially rewarding. If you can't summon a whole lot of genuine enthusiasm, then your 'game' would be pretty transparent.

You've got the 'service to others' aspect covered, so be honest about it.
