Is coag negative staph + blood culture significant?

Depends on the clinical picture. Could be a contaminant or could be legit bacteremia.

Like I said, depends on the clinical picture. Why did the patient have a blood culture? What else is going on?
 
So would you treat this?

Some of this is a matter of style. Some attendings don't really seem to believe in the "contamination" phenomenon.

Generally though if you have 2 of 2 positive for coag - staph, you'll treat. If you have a central line in place, you'll most likely treat as well, even if 1 of 2, and probably pull the line if you can.
