Has anyone ever heard of or known anyone who secured an Optho position with a 188 on Step 1. Do Step 2 scores matter if significant improvement is shown?

In your case, taking Step II very, very, very early can only help you. If you don't take it and have your score in (with substantial improvement), I'd say your chance of matching in Ophtho is virtually zero.
