Cognitive Therapy Basics and Beyond by Judith Beck is a great overall reference. If you are looking for more of a workbook (great handouts for therapy) I'd recommend Robert Leahy's Cognitive Therapy Techniques: A Practitioner's Guide.
Another that I recommend is Groups in Action: Evolution and Changes. It has a workbook with excellent therapeutic techniques and worksheets. It also has a DVD that has a real group (not actors). Several techniques are used (double bind, role-playing, transference/counter-transference). Of course this is done with very high functioning individuals, and even though the material is for groups, conceptually the same principles and techniques can still be applied on an individual basis.
