I'm wondering if anyone goes into comprehensive practice for a year or two before applying for fellowships. Is it much harder to match after a year or two post residency? Is this ever done? Thanks!
One of the attendings decided to go into oculoplastics after two years in a general practice. She's now in the second year of her fellowship. She ended up having a total of 3 years off.

Yes it can be done, but there can be expensive consequences. After more than a couple of years of practice, claims-made malpractice insurance policies mature to full-rate, and buying a reporting endorsement ("tail coverage") can get expensive. If you work for a big organization that covers this cost, or are using an occurrence-type of policy, the cost is covered. Best to be sure.
