Does your all gross in breast cancer lumpectomies, mastectomies the same day of the surgery?
If not, why?
What if there’s a case on Friday? Does the 6-72 hour predictive marker fixation time come into play at all?

We gross all breast cases same day unless it’s a super late case then it’s first thing the next day. We have a Friday cut off time. I previously worked at a place that routinely would send breast cases very late on Friday and it was always an issue.

I think the tissue fixes better if we fix the specimens overnight. We gross the cases that come in the morning to come out the next day. The cases from the afternoon we try to hold over to gross the next day. However, if they come on Thursday, we do put them through to avoid the greater than 72 hour fixation rule. If there is an obvious mass lesion, we might just take sample of the tumor and then gross the rest of the case on Friday. Cases that come on Friday get grossed on Friday or someone could come during the weekend to gross it if they wanted to let it fix longer--we don't get many breast cases on Fridays.
We only have histology M-F, so Thursday afternoon cases are the issue for us.

I almost always let it fix for 24 hours or more then gross. I admire those who gross it fresh, the squishy fat is just too hard to work with I find.
I do come in on weekends if needed.
