What's the OFFICIAL guideline accepted by both COMLEX and USMLE on breast exam? There are some conflicting info. FA says no regulation. Combank says do it from 20-40 every 3 years then once every year after 40 along with mammogram. Comquest says only once every 5 years from 20-40. Uworld says no exam until 40. I don't know what to pick now.

I'd say go w/ the USPTF guidelines:

The USPSTF recommends against teaching breast self-examination (BSE).
Grade: D recommendation.
The USPSTF concludes that the current evidence is insufficient to assess the additional benefits and harms of clinical breast examination (CBE) beyond screening mammography in women 40 years or older.
Grade: I Statement.

I would think a reasonable approach is to ask the patient if they want a breast exam (if >40yo).
