CA Verification of Licensure Form 17A-16

I'm so confused. I'm having all my preceptors signed my intern hours. But according to CA BOP website "licensing state agency" should sign the form 17A-16? Are they referring to State BOP or School which has the license to verify intern hours? Any out of state pharmacist who moved back to CA with tips to process CA application for licensure?? I am looking for advice so I can get my CA apply to go smoothly (so I can avoid deficiency) Thanks a bunch!

I think I am in the same boat as you. I just completed my 4th year of pharmacy school in New York and had my preceptors sign my affidavit forms. From what I've been seeing in previous threads is that California will only do 600 hours for APPE intern hours from those affidavit forms. If you just have your State board fill out the 17A-16 form and send it to california with 1500+ hrs documented by that State board than you will be eligible to sit for the CPJE without having to take an additional 900 hours.
 
I'm from out of state but the process was slightly different last year - just have all your out of state preceptors (or any work you've done) sign the 17A-16 form. You could also have your state send your hours to CA as well. For any hours you get done in CA, have the in-state form filled out (it's in the application packet). Once that's done, send everything to the board.
 
I am glad I was not the only one having issues with this one. After quite a bit of research and reading into the application, it has been deciphered.


So California went about adding two new things to the 2013 Pharmacist Licensure application. One was the addition of the exam security and intern hours affidavits (the one essentially stating you are not being untruthful with your hours); and the other was this verification of licensure from another state.

Before the 2013 application came along, it was not a requirement to have another state verify your hours. In fact, it was an optional additional step that could be taken but was not necessary at all. Now they have made it a requirement, and state that in EVERY state you have held an intern license you must have that board provide proof along with the number of hours attained.

It took me a long while to figure out where this extra bit came from. So yeah, you have to include it along with everything else.

The NPDB self-query thing is new as well.

Does anyone know where I could find the 17A-16 form? I can't find it on the California pharmacy license site?
 
I recently talked to the CA BOP regarding this issue and they said if you from out of state pharmacy school, you have two option, which was ever to get the affidavit to be signed off by your school, retail pharmacy, and hospital pharmacy. If you do not wish to do it this way, then you can have your state, which you have did your intern hours verified your hours and send them to the boards. The CA BOP is currently working on May 15th application.
 
So once you submit all of the application materials, how long does it take for them to approve the application?
