For this one does anyone know the sig. for the Imuran?
And what is the Drug name on the 2nd prescription?

Is this correct: Megace 240 units? 10 ml at bedtime?
Is this the sig for the Neurentin: 1 by mouth at bedtime?

On the first prescription. what would be the sig. for the Prednisone. I dont understand because it says 60 mg then 20 mg. Cytoxan says 100 mg then 50 mg.

prednisone and cytoxan..doc wants u to dispense 20mg tab for pred and 50mg tab for cytoxan

darvocet N for the second rx. Imuran looks like 3 times once a day but not sure. I would call the md for that one.

The megace is 240 ml. Take 10ml or 2 tsp at bedtime

I'll let someone more experienced take the other questions
 
1. Looks like 3 tabs per day (not 100% sure but pretty sure) and Darvocet-N 100
2. Megace, disp 240 ml, 10 cc at bedtime and yeah that's Neurontin but that doesn't come in 200mg so it'd prob be 2 caps of the 100 mg at bedtime
3. Yeah that's what I see
4. Prednisone doesn't come in 60 mg. So you dispense 20 mgs to add to the correct dose. Same with the Cytoxan, the tablet does not come in a 100 mg strength.

Yeah, what class is this?? Shouldn't you do your own homework... 😛

To me the Darvocet says ~ 5 D, so I would qs for 5 days qid or #20 tabs. In reality it would probably be good to give the doctor a call.

For the Neurontin, the doc wants #20 tabs initially with the refills set at a 2 month supply. Maybe he just wants the patient on a trial dose at first for 20 days.

i see #50 for the darvocet (MD usually spells out the number in addition to the numerals, in some states its the law)... the neurontin is #20, then prn refills for #60 (with the same sig)
 
This is exactly whats wrong. The fact that these scripts are open for interpretation just opens doors wide for errors. Why cant docs write legibly or just adopt CPOE??

Unacceptable.

They take a class in dr handwriting....

if you cant read it, then they can blame someone else (ie, the pharmacist) for their mistakes.

The same reason why you cant get a real signature on the Rx. Hopefully, you will fill it under another dr's name.

Always be covering....:laugh:

This would be a good teaching exercise if it was showing why ISMP and JCAHO have compiled unsafe abbreviation lists...otherwise it's just teaching students how to make somewhat educated guesses when the phone should be picked up instead.

None of these were difficult to decipher. Only one of these would require calling for clarification. Z could fill all of them and hasn't done retail in a 100 years. They are all actually pretty clear except for the Neurontin.. It requires you know drugs and dosages.

I like it better when I write orders (which is 100% of the time). Have not verified one in years....

There wasn't a thing wrong with any of those. I see a 500 scripts a week that look no different than these.

The only one that needs clarification is the Neurontin. I have seen it written as two scripts before. One RX for the titration and another for the maintenance regime. It looks like that is what the Doc was going for except there is no SIG for the quantity of 20.
