Can you get into a competitive residency (like ophthal) with an H1B visa?

Maybe. There's no way to be certain. International students at US schools on F visas are relatively uncommon. I expect many Derm/Ophthal programs only see one every few years, if that. You'd need all the other fixings -- excellent USMLE scores, research, well ranked in your class, etc. Getting a research year might be more complicated as a non-citizen -- you can't be paid by many goverment research grants, for example, and you'd need to find a legal way to do it on your student visa (i.e. you can't just be "hired").

And then you'll need a prelim year, so you'll actually need to match into two programs that are happy with an H visa.

And there's multiple years from now until then, and visa laws or policies may change.

And finally, you haven't even started yet. You may not even want these fields.

So, maybe.

I don't know the answer to that, I just wanted to say that it really annoys me when people say things like "ophthal"
