Canadian in U.S. med school - question about the VISA issue

I'm just starting medical school in the U.S., and I have questions about applying to U.S. residencies and the VISA issues. Hopefully someone can help me out. Do Canadians have to apply for the H1 VISA to work as residents in the US? Or is J1 a better deal? If the residency program states that they do not sponsor H1's, is there a way around that? Thanks in advance.
