What’s the prognosis?
How geriatric? Age/frailty/comorbidity?
What’s being secreted?
 
Rather not go into patient specifics, other than living past original prognosis by 3 years, 70's for age, and higher functioning, that I hope to be once I'm in that age range. Not frail. On lineotride for carcinoid syndrome.

Post purpose, more of a generic question for approach others have had with condition rather than hammering in best option for this pt.
 
My experience has ranged from octreotide being helpful if it’s serotonin producing(and excess serotonin causing agitation/anxiety) to benzos to nothing being helpful to the point of palliative sedation. Or the anxiety is not due to carcinoid but rather more typical anxiety associated with a serious illness. Sometimes I’ll avoid serotonergic meds, sometimes you don’t need to.

Approach: explore what it means to live past prognosis, figure out what the tumor is secreting, go from there.
