Does anyone know what the exact guidelines are for cardiac surgical patients beta blockade prior to and the day of surgery? Can you tell me where to find the information?
Which guidelines are you referring to? SCIP wants patient on beta blocker to receive it perioperatively (at least within 24 hours preop or in OR) or documented why not given. ACC/AHA have all sorts of guidelines, but I don't think they relate to just cardiac surgery patients with any of them.
STS guidelines for CABG are that all isolated CABG patients must receive *a* beta blocker within the day prior to surgery.
Keep in mind there is no evidence to support this as a quality metric, and there is evidence that de novo beta blockade the day of noncardiac surgery is harmful.
So what do we do to get around this absurd regulatory contradiction? 1cc of esmolol. Voila! Quality!
