I was wondering if there are any direct cardiology programs so that one doesn't have to go into internal medicine first and then apply for a cardiology fellowship. And if they do exist, does anybody know how many and how difficult they are to get into?

No, Cardiology remains an IM fellowship. Every so often, people talk about short-tracking cards fellows (2 years of IM, then onward) but its not close to happening.

I have a friend in Spain who is doing cardiology. Over there it is a 6 year residency after medical school, and you only have to do one year of general internal medicine (which counts as your first of 6 years).
