This is a tough question, I would say it's difficult to rank, but in terms of research/prestige I think a few stand out and it's difficult to separate amongst them:
Cleveland Clinic (? after Topol leaving what will happen), BWH, MGH, Mayo Clinic, Hopkins, Stanford, Texas Heart, Emory, Cornell, Duke.
