I'm confused with the CAS pricing system. Suppose I want to apply to 60 programs, would I pay 60x35=2100 or would I pay 60+100+150+200+(20x35)=1210? If it's the first, it makes no sense to initially apply to 60 programs as it would be better to apply to 40 (at $20/program) and then add the other 20 programs (at $35/program) as add-ons right after submission. Any insight would be much appreciated.

I was confused by this as well last year. It's the same pricing basically as the NRMP system that you submit for PGY1 programs. I.e. it's your latter example, the cheaper one.
