I hope to write my first case report. please help me get an approach ?

Usually there is not much too it.

The Case described in detail with all relevant findings
The Discussion which is a brief review of the literature and how your case adds knowledge to that literature.

That's about it.

Once you've decided on the journal you will submit to, look up recent case reports published in the journal and pay attention to the length, format, level of detail, and so forth. Review the journal's instructions to authors for other information about formatting the case report. Then go for it.
