Cash Based Physical Therapy Referrals

Hey everyone,

I'm new to this forum, but I came across a thread from a psychiatrist asking how to get referrals from other physicians, and there was a lot of great advice, so I thought I'd ask a similar question.

I'm a physical therapist who owns a cash-based outpatient practice in Green Bay, WI. I specialize in sports injury prevention and rehabilitation, with a niche in helping golfers through movement assessments, rehab, and golf swing analysis.

I've tried reaching out to physicians in my area, but it's been difficult to build referral relationships. I understand that cash-based PT is a little different from the traditional insurance model, and I'm sure many physicians already have established referral patterns.

For those of you who do refer patients outside your usual network, what actually gets your attention? What's the best way for someone like me to introduce myself? Is an email enough, or do you prefer an in-person visit? Are there things that immediately make you more or less likely to refer to a PT practice?

I'd really appreciate hearing from physicians, APPs, or anyone involved in referral decisions. I'm not looking for a sales pitch—I'm genuinely trying to understand what is valuable from your perspective so I can build relationships the right way.

I'm going to be very honest - I don't think that your problem is that many of the physicians in your area already have established referral patterns. What you are offering is way too niche for the average outpatient family medicine physician.

I can count on one hand the number of times I have referred a patient to a cash-based practice. So few of my patients would be willing to pay cash for any service, much less PT. And much much less PT that focuses on sports injury and prevention.

Are there any concierge (not DPC) practices in your area? Are there enough patients in your area who have the time, resources, and motivation to see a cash based PT practice that will help them work on their golf swing? You could probably do very well in parts of Southeast Florida, or even the particular suburb where I live in Michigan, but I don't know Green Bay that well.

I think you're probably right that I'm not the right fit for the average primary care patient. My marketing is definitely aimed more toward active adults, recreational athletes, and golfers who are motivated to stay active and are willing to invest in their health.

The golf swing analysis is really just one part of what I do. I also treat common orthopedic issues like shoulder, knee, hip, neck, and back pain, with an emphasis on helping people return to the activities they enjoy. The sports injury prevention aspect is geared toward keeping active people doing what they love rather than waiting until they have a major injury.

I guess my question is more about the subset of patients who would be a good fit. For those patients, what would make you comfortable referring to a cash-based PT versus a traditional PT clinic? Is it outcomes, communication, availability, patient feedback, or something else?
