Chances of Intern year and Residency at same place?

Correct me if I'm wrong, but for some specialties, you're going to have to apply for both an internship (PGY-1) and the residency during MS4. How common is it that you get both programs at the same institution/hospital? Or do people generally do it at 2 different places?

That's tough to give an exact answer. Some advanced programs chat with the prelim programs (at the same hospital) in order to have people high on the advanced rank list to also be high on the prelim rank list. In addition, you make a separate prelim rank list for each advanced program you rank, so you have the ability to maximize your chances of matching to a prelim and advanced program in the same hospital/town.

Having just gone through this, I can say that while possible and maybe even "likely" it isn't a for sure thing. It really depends on a few things: The specialty how competitive it is and how many programs there are, which cities you are targeting ie big vs small, and finally how many places you are willing to interview.

Based on those factors it can change a LOT. For example, lets say you are applying to dermatology, want to live in a smaller city and aren't willing to go on a ton of extra interviews. Well derm is competitive with few programs. So, you would have a LOW chance in this scenario.

However, if you are applying to PMR, want to live in a big city, and are willing to go on as many interviews as it takes then you have a HIGH chance of moving once.

Also, some programs do talk to the prelim med program at their hospital, but even this isn't a for sure thing.

I was literally just talking about a classmate about this today.

It seems to vary from program to program. Unfortunately, there's no way of knowing...I know a guy that matched at a very competitive rads program but didn't match at their TY (not exceptionally cush or anything). It's so weird how that stuff works out.

Really depends on the advanced specialty that you're going into. Some TY's can be pretty competitive, however, if you're going for a pre-lim and a difficult PGY-2 specialty, that should be somewhat doable, depending on the location. Obviously nothing in set in stone, although there have been more programs (at least in Rad Onc) that have an integrated PGY-1.
 
Not really sure what specialty I'd apply for, but I was looking into PM&R. It seems it needs a prelim year in internal medicine, surgery, peds, or family medicine. I thought people can only do internships in IM or general surgery. How do you do pediatrics or FM as your PGY-1? Do you get in then just "quit" after a year? Or are some programs designed for only 1 year?

I would say for a 1-year internship, the desirability goes TY >>> Pre-lim IM >>>>>>>>>>>>>>>>>>>> Pre-lim surgery. Unless you're going into a surgical field.

Generally but not always true. At seem places the prelim med is just as cake as the TY. And maybe even more so, as 6 hr days on a random medicine elective beats the 75hr gen surg month that every TY requires.

My peds residency has some prelim spots and they are all filled by people who are straight out of school and have advanced residencies lined up. One plans to do peds optho, the others plan to do adult stuff but liked the program and/or location.

That's wrong -- agree with the previous post. There aren't many prelims outside if IM and surgery, but they exist, and not for career changers.

As to the OP, for advanced paths you generally need to line up the prelim/transitional year and advanced program separately. If your advanced is in a big city, then there are probably multiple prelims in town. Or you may want a very cushy transitional program that may not be located anywhere near your advanced program. What you don't want to do is just try to do a prelim at the same place as the advanced only because of the convenience, without really checking out that program independently. Often that's how you end up in something malignant and brutal for a year. You can often find something much more benign across town.

No, there are (very few) FM or peds programs with designated preliminary spots - in other words, they are spots where they know that someone will only be there for a year before doing something else.

The peds prelim spots are sometimes designated for people doing special residencies - like combined peds + psych or peds + neurology.

The only FM preliminary spots that I am aware of are offered at military residencies, and they are to prepare people for their General Medical Officer tour. They are generally offered at military hospitals.

There are a few OB 1-year preliminary spots, but they are generally for people who plan on doing OB but did not match, so they will try for the preliminary spots as a "holding pattern" while they re-apply for the match.

True, I imagine there are some medicine pre-lims that are easier. However, unless you talk to people that have been through those programs, it's likely hard to determine which ones are the outliers.
