Change at end of PGY1

If an intern decided they wanted to move from their current program at the end of PGY1 what's the best course of action?

Is it best to wait until the very last moment to go and see the PD?

At the end of the internship year what happens? Do all programs issue a certificate of completion?
 
i think it is easier if you are up front with the pd as soon as you are sure you are going to switch. i was in the same situation last year, and tried not to tell the pd for as long as i could (on the advice of someone else who experienced the same situation), but i was so unhappy in the program that it really showed and my dishonesty seemed to be creating more problems. initially after i told the pd there was some friction, but it ended up being better in the long run b/c they knew where i stood (i.e. i didn't have to be secretive about my plans) and they were also able to find a replacement for me. so, i think it is better to be up front with the pd that you are leaving. also, you do get a certificate of completion of PGY-1
 
Timing for a decision like this can be difficult. On one hand, you will need a letter of recommendation or a letter of good standing from your program director. Your PD can also help you find another program and put you in touch with PDs in other fields. If you plan on going through the match again, it would be advantageous to tell the PD early in the year.

If you are a categorical resident, you should let the PD know sooner rather than later, so they can fill your spot. It can be difficult on your fellow residents if a program is short the next year. There are plenty of people looking to get into another field, and by declaring early, it may open a space for someone who really wants it.

There can be some friction after letting the PD know. In my program, it is usually from the chief residents who either think you are weak or wasting their time. My PD, at least superficially, wants the residents to be happy, whatever field they choose.
